Blazek Event Center Rental Agreement & Policies

If alcohol is being served, the following waiver must be signed. The Blazek Event Center, Inc. will not supply any type of alcohol, but you may self-serve or hire a third party to serve alcohol as long as we have a signed waiver.

LIABILITY WAIVER — ALCOHOL

The Client acknowledges and agrees that Blazek Event Center, Inc. is in no way liable for any incidents arising from alcohol consumption, including but not limited to:

Over-serving of guests
Drinking and driving
Health risks or complications
Injury or damage related to intoxication

The Client accepts full responsibility for guests’ behavior and compliance with applicable laws.

FINANCIAL TERMS

Deposit $100 Due upon signing
Rental Fee Due 7 days prior to event

Deposit Policy

Full refund if canceled more than 7 days before event
Forfeiture if canceled less than 7 days prior to event

ADDITIONAL CONDITIONS

Set-Up/Clean-Up: Must occur within the reserved time window
Damages: Client liable for any damage incurred
Cleaning Fee: May apply if space is not returned to original condition
Decor Restrictions: No adhesives that harm surfaces, nothing that puts holes in the wall
Noise Compliance: Abide by local ordinances
Insurance: May be required at Blazek’s discretion
